Deferred Compensation

As an employee of the State of Vermont, you are eligible to participate in a long-term retirement investment program known as the Deferred Compensation Plan. You may elect to defer part of your wages for the purpose of contributing a Sec. 457(b) Pension Plan as allowed by Internal Revenue Service regulations and which the Board of Trustees administraters for the State of Vermont Retirement System.

If you are requesting a deduction for the first time, please contact the Great West Retirement Services Plan Administrator, at 802-229-2391 before you submit this form.

 

Delegation of Authority

Completion of this form authorizes a person to approve and sign personnel forms and expediture documents on behalf of an organization.

 

Direct Deposit

An Employee of The State of Vermont is required to directly deposit his or her salary to his or her account with a financial institution. The form requires the bank name and address, bank transit routing number, employees account number and specification of checking or savings account.
 
Please note, all finiancial institutions, except the Vermont State Employees Credit Union, require pre-notification before funds are sent. This will delay your direct deposit by one pay period.
 

Form W-5

Use Form W-5 if you are eligible to get part of the earned income credit (EIC) in advance with your pay and choose to do so. See the form itself to see who is eligible to get advance EIC payments.

 

Salary Advance

This form is intended for use only for Salary Advance requests submitted on behalf of the employee by their personnel officer. Salary Advance requests should follow the procedures outlined in Policy 12.10 Employees Request for Salary Advance.

 

Special Check Request

This form is intended for use only for special check requests submitted on behalf of the employee by their personnel officer to address errors or omissions on the part of the State that need to be corrected on behalf of the employee. Actual Salary Advance requests should follow the procedures outlined in Policy 12.10 Employees Request for Salary Advance.
