EXPENSE USER INFORMATION
Required for all requests. The information below is for the person whose security needs to be added, changed, or deleted.
USER CHARTFIELD INFORMATION
Complete this section when requesting new users and chartfield changes.
CHANGE TO USER'S SUPERVISOR
Complete this section when requesting a change to an existing user's supervisor. (This section does not need to be completed if a new supervisor has been requested and the names of the employees who report to this supervisor were included on the request form.)
SUPERVISOR'S APPROVAL LIST
Complete this section when adding a new user who will be a Supervisor. Enter the names and the employee numbers of who the new user will be supervising.
